Job Title: Locum Senior Physiotherapist

About the Role:
We are seeking an experienced Physiotherapist to join the innovative transiptional care programme, an initiative designed to support older adults as they transition from acute care to community-based services. This role involves working closely with a multidisciplinary team to deliver quality, person-centred care, enabling clients to safely reintegrate into the community with improved mobility and independence.

Key Responsibilities:

• Provide tailored physiotherapy interventions, including assessment, planning, treatment, and evaluation, to help older adults regain function and mobility post-acute care.
• Collaborate with an interdisciplinary team to develop personalised rehabilitation and transition plans for clients.
• Utilise evidence-based practices to optimise client outcomes, promoting independence, safety, and quality of life.
• Maintain clear and accurate documentation, adhering to best practice standards in aged care and rehabilitation.

What We're Looking For:

• AHPRA-registered Physiotherapist with AHP3 level experience (4+ years experience)
• Demonstrated experience in a similar setting, ideally within aged care, rehabilitation, or community transition.
• Strong interpersonal and communication skills for effective collaboration within a multidisciplinary team.
• Ability to work independently and efficiently in a dynamic healthcare environment.

Benefits:

• Competitive hourly rate: $67 - $72 per hour.
• Full-time hours, Monday to Friday.
