Job Title: Senior Occupational Therapist- Aged Care and Community

About the Role:
We are seeking an experienced Occupational Therapist to join the pioneering programme as part of a TCP with a Local Health Contract. This new initiative is dedicated to supporting older adults as they transition from acute care to community-based services, with a focus on quality, continuity, and compassionate care. In this role, you will collaborate with a multidisciplinary team to help clients achieve optimal outcomes and facilitate a smooth transition to ongoing care in the community.

Key Responsibilities:

• Provide comprehensive occupational therapy services, including assessment, planning, intervention, and evaluation, to older adults transitioning from acute to ongoing care.
• Collaborate with an interdisciplinary team to create personalised care plans that meet clients' individual needs.
• Utilise evidence-based practices to enhance client independence, quality of life, and safe community reintegration.
• Maintain accurate documentation and adhere to best practice standards in aged care and rehabilitation.

What We're Looking For:

• AHPRA-registered Occupational Therapist with AHP3 level experience.
• Proven experience in a similar setting, ideally within aged care, rehabilitation, or community transition.
• Strong communication and interpersonal skills to effectively collaborate within a multidisciplinary team.
• Ability to work independently and efficiently in a fast-paced environment.

Benefits:

• Competitive hourly rate: $67 - $72 per hour.
• Full-time hours, Monday to Friday.
