You'll be the heartbeat of an aged care facility, partnering with residents from admission to discharge and ensuring they receive top-notch care tailored to their unique needs and goals.

What You'll Do:

• Create a welcoming environment for new residents and their loved ones on admission day.
• Collaborate with the Clinical Nurse Manager to ensure a smooth and supportive experience for everyone.
• Conduct initial assessments and develop personalised care plans that reflect each resident's aspirations.
• Organise and facilitate engaging Resident Review Meetings.
• Be the go-to contact for staff and families, addressing any care needs and concerns with compassion and expertise.
• Champion meaningful activities that build social connections and community involvement for residents.

Who You Are:

• A current AHPRA Registered Nurse with extensive experience in aged care (bonus points for skills in wound care, dementia, or palliative care!).
• A customer service superstar with a genuine passion for the needs and desires of older adults.
• A strong communicator who builds relationships easily and knows how to bring out the best in others.
• Organised, with excellent time management and decision-making skills.
• A team player who thrives in a positive and collaborative environment!
